Dear Honorable I am writing you to letter to inform you that my son Jeremy Slaton has had another
stroke which happened on 4/2/2023. Jeremy is not able to do for himself at this present time. Hi
doctors feel that Jeremy needs to be place in a rehabilitation Center so that he can get threpory that he
needs. Right now I have to bath him and help him get dress wellcare that my son insurance company is
working on finding a center that has a bed available that is in close range to his dialysis center. My son
wilt remain their until I get back home. Wellcare is telling me it normally takes them 2 to 4 weeks to
place a patient. I just wanted to give you and your staff an update on this matter. I asked Jeremy
Doctor because he knows about my situation of how lone do I need to before I self-surrender and he
told me that defiantly by to second week in may Jeremy will be in a rehap center. So I am humbling
requesting that my date be moved to accommodate my son being moved to a center where he can
receive the care that he needs while I am away.
