Hey got your voicemail and yes I review the packet before submission. The following items are needed to make
the packet complete:
1. Application – this is the borrower application (attached) The one that was submitted was filled out by Jill Sport,
initialed and signed. This needs to be initialed and signed by you as the borrower.
2. Date of Business established
3. Owner information – please refer to checklist attached for exact information needed for this. They are particular
even down to the last 4 digits of you zip.
4. NAICS code
5. Note or spreadsheet of your calculation for how the loan amount was calculated. I have the Schedule C which I can
see the numbers Jill used but just need something from you simply showing the math.
6. Proposed Use of Proceeds – this is okay except for Jill’s signature. She is not an authorized agent for the bank
therefore she should not sign. Please redo this form and only you sign it.
