Defendant adjudicated guilty as to Counts One, Four, Eleven, Twelve, and 
Thirteen of the Indictment. 
 
SENTENCE IMPOSED as to Counts One, Four, Eleven, Twelve, and 
Thirteen of the Indictment. 
 
IMPRISONMENT: SIXTY (60) MONTHS. The term consists of 
Twelve-Months as to each of Counts One, Eleven, and Thirteen and 
Twenty-Four-Months as to each of Counts Four and Twelve. The terms 
of imprisonment in Counts One, Eleven, and Thirteen, shall run 
concurrently with each other and Counts Four and Twelve will run 
consecutively with each other and run consecutively to all other 
counts. 
 
The Court recommends to the Bureau of Prisons that the defendant be 
incarcerated as close to Jacksonville, FL as possible at the lowest available 
security designation, enroll in a residential substance abuse treatment 
program (RDAP), receive mental health programming, and any vocational and 
educational programming available.

SUPERVISED RELEASE: THREE (3) YEARS. The term consists 
of Three years as to Counts One, Eleven, and Thirteen and a One year 
term as to Counts Four and Twelve, all such terms to run 
concurrently. 
 
 
Special conditions of supervised release: 
 
 
Mandatory drug testing requirements are imposed.  
 
The defendant shall participate in a substance abuse program 
(outpatient and/or inpatient). 
 
The defendant shall participate in a mental health treatment program 
(outpatient and/or inpatient). 
 
The defendant shall be prohibited from incurring new credit charges, 
opening additional lines of credit, or making an obligation for any 
major purchases without approval of the Probation Officer. 
 
The defendant shall provide the probation officer access to any 
requested financial information. 
 
Defendant shall cooperate in the collection of DNA. 
 
 
SPECIAL ASSESSMENT of $500.00 is due immediately. 
 
FINE and cost of imprisonment are waived. 
 
RESTITUTION TOTAL AMOUNT $363,099.29. 
 
FORFEITURE: The defendant shall forfeit the defendant’s interest in the 
assets identified in the Preliminary Order of Forfeiture (Doc. 31). 
 
Counts Two, Three, Five through Ten, and Fourteen through Twenty are 
dismissed on the motion of the United States. 
 
The defendant is remanded to the custody of the United States Marshal. 
 
Defendant advised of right to appeal.
