EVIDENCE REGARDING CLAIM 
SUBMISSIONS AND REQUESTS FOR 
EXCLUSION 
 
The deadline for Class Members to submit a claim form was April 28, 2022, and the 
deadline to submit a request for exclusion from the class was March 4, 2022.  By no later than 
May 9, 2022, Plaintiffs shall submit a declaration by the Settlement Administrator that provides 
updated information about the number of claim forms submitted by the April 28, 2022 deadline 
and the estimated claims rate, as well as the number of timely requests for exclusion.
