CAROLYN DENISE WADE Defendant. / AFFIDAVIT OF DEFENDANT AS TO MOTION PURSUANT TO FEDERAL RULE OF APPELLATE PROCEDURE 24 TO PROCEED IN FORMA PAUPERIS ON APPEAL I, Carolyn Denise Wade, have read the Motion Pursuant to Federal Rule of Appellate Procedure 24 to Proceed In Forma Pauperis On Appeal prepared by my attorney. The facts stated therein are true and correct. I prepared the CJA-23 Financial Affidavit form and the financial information therein is true and correct. I am indigent and do not have the ability to pay for transcripts and filing fees for an appeal. I wish to exercise my constitutional right to appeal and therefore claim an entitlement to redress. On appeal, I intend to raise the issue that the evidence was insufficient to prove that I submitted or directed anyone to submit false information to the government in order to receive PPP loans. I will also raise any other issues my appellate counsel identifies as meritorious after his review of the transcripts and record.
