JANET T. MILLS, in her official capacity as Governor of the State of Maine, JEANNE 
M. LAMBREW, in her official capacity as Commissioner of the Maine Department of
Health and Human Services, NIRAV D. SHAH, in his official capacity as Director for
the Maine Center for Disease Control and Prevention, MAINEHEALTH, GENESIS
HEALTHCARE OF MAINE, LLC, GENESIS HEALTHCARE, LLC, NORTHERN
LIGHT HEALTH FOUNDATION, MAINEGENERAL HEALTH,
Respondents. 
I, Roger K. Gannam, am over the age of eighteen years, and upon my oath
depose and say that: 
I am an attorney of record for Applicants, and that on this 20th day of
October, 2021, I served the within Emergency Application for Writ of Injunction and
Appendix of Exhibits in the above-captioned cause upon all counsel of record by 
submitting an electronic version to the e-mail addresses for counsel of record for
Respondents, in accordance with this Court’s April 15, 2020 Order relieving parties 
of the obligation to effectuate service by paper copies;
That on the same day as above, and in accordance with this Court’s April 15, 
2020 Order, I dispatched a true and correct copy of the foregoing Emergency 
Application and Appendix for hand-delivery to the Supreme Court Clerk’s Office; 
and
That, in addition, the Emergency Application and Appendix have been
electronically filed through the Court’s electronic filing system.
I declare under penalty of perjury under the laws of the United States that
the foregoing statements are true and correct.
